Wysięk w ranie zapewnia nie tylko odpowiednie pH, ale dostarcza też elektrolity, przeciwciała i białka. Rana wysycha, kiedy nie pojawia się wysięk lub jest go zbyt mało. Do nadmiernego wysychania rany może doprowadzić między innymi nieodpowiednia, zbyt agresywna pielęgnacja lub obecność martwicy spowodowana zakażeniem.
Najczęstszą przyczyną wysychania ran jest nieprawidłowy dobór opatrunku w stosunku do ilości wysięku. Jeżeli na ranę o niewielkim wysięku zastosuje się opatrunek o dużej chłonności, to zaabsorbuje on cały płyn, doprowadzając do wysychania rany. Kolejną przyczyną wysychania rany jest kontakt rany z powietrzem. Również brak odpowiedniej pielęgnacji rany w postaci przemywania może doprowadzać do spadku wilgotności.
Sucha rana często przybiera kolor żółty przez obecność włóknika, czarny, gdy pojawiają się zmiany martwicze lub zielono-żółty, kiedy dochodzi do zakażenia rany. Brak wysięku doprowadza do zwłóknienia ziarniny na dnie rany i zahamowania naskórkowania na jej brzegach. Dodatkowo na suchej ranie szybciej tworzy się strup. Istnieje wtedy ryzyko, że przy zmianie opatrunku strup zostanie zerwany wraz z nowopowstałymi komórkami i dojdzie do krwawienia. Nadmierne wysychanie rany wydłuża proces gojenia i zwiększa ryzyko zakażenia, dlatego zapewnienie odpowiedniego nawilżenia stanowi jedną z podstaw szybkiego leczenia ran.
Jak dbać o nawilżenie rany?
W leczeniu ran suchych zaleca się stosowanie opatrunków hydrożelowych o działaniu przeciwbakteryjnym. Zapewniają one nie tylko wilgotne środowisko wewnątrz rany, ale też zapobiegają powstawaniu zakażenia. Innym rodzajem zalecanych opatrunków są opatrunki płucząco-absorpcyjne z zawartością PHMB.
Ważne jest, aby oceniać stan rany przy każdej zmianie opatrunku, ponieważ wraz z procesem leczenia, może pojawić się potrzeba zmiany rodzaju opatrunku. Opatrunki powinny być zmieniane regularnie według zaleceń lekarza, aby nie dopuszczać do ich przylgnięcia do rany.
Warto też stosować maści enzymatyczne pomagające w oczyszczeniu rany ze zmian martwiczych, wilgotne opatrunki gazowe albo żele wodne. Zalecane jest również stosowanie miodów medycznych.
W przypadku ran suchych pokrytych biofilmem zaleca się stosowanie żeli antybakteryjnych, opatrunków antybakteryjnych siatkowanych lub zawierających włókna poliakrylanu oraz hydrowłókna zawierające srebro.
